HOSPICE...QUALITY END-OF-LIFE CARE
Hospice is considered to be the best model for quality, compassionate care for people with a life limiting illness. By involving Hospice early when a terminal illness exists, we can help to better manage a patient’s pain and symptoms. The Hospice team engages family members in the care and support of the patient, training them in caring techniques, encouraging open communication among the family members and patient. We help both the family and patient with the issues of the illness and the reality of death. Waiting to involve us when time is short often decreases a patient’s options for comfort and takes away quality time that family members would have otherwise enjoyed.

A CIRCLE OF CARE
A Registered Nurse and Licensed Social Worker will make the initial visit to your home to help establish what you may need in the way of spiritual, educational, financial, bereavement or other support. Once this determination has been made, they will work to build a team that will develop a plan of care. 

This team involves the patient and their physician, pharmacist, registered nurses, medical director, aides, chaplain, volunteers, family and others who will be providing at-home care, which includes nursing home, with the care and support to both the patient and family.

     as necessary to maintain a patient’s
     at-home comfort
  • Short-term inpatient and respite care

     related to the terminal illness
  • Provide medications for symptom
     management and pain control related
     to the terminal illness
  • Hospice care continues following patient’s death - specially trained bereavement  
     counselors help the surviving family members deal with their grief through
     visits, written communication and support programs
  • Care in nursing homes

HOSPICE CARE EVERY STEP OF THE WAY
Hospice uses a team approach and you and your family/caregiver are active members of the Hospice Care team. Team members inlclude:

  • Patient’s Personal Physician
  • Pharmacists
  • Hospice Medical Director
  • Registered Nurses                                    
  • Certified Nursing Assistants
  • Licensed Social Workers
  • Clergy, Chaplain or Other Counselors
  • Trained Volunteers
  • Speech, Physical or Occupational Therapists (if needed)

ALWAYS AVAILABLE
Hospice support is available 24 hours a day, 7 days a week to provide care and comfort to people in the early to late stages of a terminal illness. Whether a patient needs physical, spiritual, or psychosocial support, the goal is to keep them as comfortable and alert as possible to maintain their quality of life.

Quality of life is important to each of us. Due to illness, or simply the passing of time, none of us know when we’ll need help to maintain that quality.
